Hyannis, MA (April 14, 2022) - Cape Cod Healthcare is conducting a Community Health Needs Assessment to learn about the health of people living in Barnstable County. As part of this project, a Community Health Survey is available to all to identify how people’s healthcare needs are currently being met. Information collected through this survey will also be helpful to evaluate which programs or services may be needed in the future. Feedback from this survey will be helpful as Cape Cod Healthcare completes the three-year Community Health Needs Assessment for 2023-2025. The Community Health survey is anonymous and doesn’t require a participant’s name or address. This survey takes about 15 minutes to complete. This survey is open through May 13, 2022, and is available in four languages: English, Portuguese, Spanish and Haitian Creole.
