Community Health Center of Cape Cod has received recognition from the Massachusetts Health Policy Commission for its integration as a Patient Centered Medical Home (PCMH) and Behavioral Health Services Provider. The State award compliments its other recognition through the National Council for Quality Assurance (NCQA) achieved last month. CHC is currently one of only a handful of health care providers who have achieved this level of medical and behavioral health integration. Standards for the NCQA Patient-Centered Medical Home (PCMH) emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement. CHC received Level 3 Recognition, the highest level possible, as they did in 2013 when they were last eligible for the recognition. "It's an honor to again be recognized for our quality of care. It's something we strive to improve upon daily," says Chief Executive Officer Karen Gardner. "This distinction is significant, because it tells us that our patients' medical needs are being met." Ms. Gardner credited the providers and staff at CHC for their commitment to matching the rigorous PCMH standards. According to the NCQA, PCMH Recognition is the most widely-adopted model for converting primary care practices into medical homes. Research suggests that medical homes can have higher quality and lower costs, and may improve patients' and providers' experience of care.
