Please be advised that all Cape Cod Healthcare community pharmacies are accepting TRICARE insurance and will be glad to assist beneficiaries who wish to transfer prescriptions from CVS pharmacies. CVS recently announced that as of Dec. 1, 2016, its pharmacies, including CVS pharmacies located within Target retail stores, will no longer be included in the TRICARE network. TRICARE is the insurance of the U.S. Department of Defense military health system. CCHC community pharmacies are located at: • Fontaine Outpatient Center - 525 Long Pond Drive, Harwich. Telephone: 774-338-5010 Pharmacists at all three CCHC community pharmacies will gladly assist TRICARE members who wish to transfer their prescriptions to a CCHC pharmacy. TRICARE beneficiaries can transfer prescriptions by: • Taking their prescription bottles to the pharmacy and asking the pharmacist to contact their former pharmacy for a transfer • Calling their CCHC pharmacy of choice and asking the staff to obtain their prescription information from their former pharmacy • Asking their prescribers to send their prescription information to their CCHC pharmacy of choice.
