Falmouth – Community Health Center is delighted to announce that it received a capacity-building grant from The Falmouth Fund of the Cape Cod Foundation to address Social Determinants of Health in Falmouth. This project is essential to the mission of Community Health Center, to improve the health of the community through the provision of comprehensive, high quality, compassionate health care to all, regardless of ability to pay. The social determinants of health CHC can address through this program are conditions where people are born, live, learn, work, play, and age that affect a wide range of health, functioning, and quality-of-life outcomes, and contribute to wide health inequities. This includes economic stability; education access and quality; health care access and quality; neighborhood and built environment; and social and community context. By providing support and resources outside of the typical health care umbrella, Community Health Center can greatly impact the Falmouth community and improve the overall well-being of patients, while directly impacting their mission to improve the health of the community. Access to these vital services would not be possible without the generous support from The Falmouth Fund of the Cape Cod Foundation, and from other individual and foundation donations. CHC cares for more than 25,000 patients, nearly 8,000 of whom are Falmouth residents. With the 2022 grants, The Falmouth Fund of the Cape Cod Foundation also reached a significant milestone: the investment of more than $1 million back into the community over the past 13 years. They have supported 90 organizations doing important work in health and human services; education, youth, and recreation; arts and culture; and the environment.
