(Hyannis, November 12, 2018) Cape Cod Healthcare is conducting a Community Health Needs Assessment (CHNA) to learn about the health of people living in Barnstable County. As part of this project, a community health survey is now available to Barnstable County residents to identify how people’s health and social needs are currently being met. Information collected through this survey will also be helpful to evaluate which programs or services may be needed in the future. “As a region, Cape Cod has unique needs. The survey will give us vital information about the kinds of health and social services that are important to residents, today and into the future,” said Terri Ahern, CCHC senior vice president, strategy and government affairs. “It’s a great opportunity for residents to contribute to identifying the issues that will be prioritized in the upcoming community health needs assessment report.” The survey is anonymous. It does not ask for your name or address, and takes about 15 minutes to complete. The survey is open from November 12, 2018 to December 31, 2018. It is available in three languages: English, Portuguese and Spanish. To take the survey or visit www.capecodhealth.org/survey. If you have questions about this survey or would like a paper version of the survey mailed to you, please email communitybenefits@capecodhealth.org or call 774-470-5511. Cape Cod Healthcare has engaged Health Resource in Action, a Boston-based public health research firm, to administer and analyze data collected through this survey.
